Securing Affidavit
ABOUT THE SERVICE
Self/Joint Legitimation is issued to individuals whose parents missed to register their child to the Local Civil Registrar are required to execute an affidavit. Late registration to child born out of wedlock must also execute a Self/Joint Affidavit of Legitimation.
Affidavit of Acknowledgement of Paternity is issued to illegitimate child whose father acknowledges the certain child.
An affidavit also is required to persons who have clerical errors in their names are required an affidavit of two disinterested persons for correction of clerical error.
Affidavit of Loss is also executed for loss items.
REQUIREMENTS
Affidavit of Legitimation
- Official receipt of payment
- Marriage certificate
- Community tax certificate of parents
Self/Joint Affidavit for Late Registration of Birth
- Official receipt of payment
- Community tax of affiant
- Community tax of two witnesses
- Filled – up form duly signed by LCR representative
Affidavit of Acknowledgement of Paternity
- Affidavit receipt
- Community tax of affiant (father)
Affidavit for 2 Disinterested Persons for Correction of Clerical Error
- Official Receipt
- Community tax of affiant ( 2 witnesses)
- A document showing the “clerical Error” data
Affidavit of Loss
- Official Receipt – community tax of affiant
