Securing Referral for Financial Assistance

ABOUT THE SERVICE
Municipal Social Welfare and Development Office assists indigent families from the barangays of the Municipality of Tubigon to provide financial assistance during emergency crisis situation needed by the client.

REQUIREMENTS

  • Supporting papers pertaining to assistance needed (hospital bill, doctor’s prescription)
  • Barangay Certification
  • Solicitation Form from SB Member
  • Mayor’s Recommendation

Submit Requirements
Client bring certificate of indigency issued by the barangay and supporting documents such as their hospital bill and medicine prescription and approach the person in charge for assessment. 1 minute MA. LOURDES B. GIMENA

Final Assessment

Secure Solicitation Form from SB Office and Mayor’s Recommendation
The barangay certification and SWO assessment report is given to client. Proceed to SB Office for the Solicitation Form. Let the SB Members sign and secure Mayor’s Recommendation. 15 minutes ENGR. WILLIAM R. JAO

Processing
