Securing Senior Citizens ID Cards
ABOUT THE SERVICE
Municipal Social Welfare Office assists elderly persons in securing their senior citizens identification cards. The id cards are needed for availing their 20% discounts on medicines, transportation and basic commodities.
REQUIREMENTS
- 2 pcs. 1 by 1 ID Pictures
- Photocopy of Birth Certificate
- Accomplished Application Form
