Securing a Medical Certificate




ABOUT THE SERVICE
Firms and government agencies may require Health Certificates from certain persons. This is especially true for seeking employment and for insurance purposes.

Schools also require students to secure a Medical Certificate before they are allowed to enrol.

REQUIREMENTS
For Student, Employment and Others Purposes:

  • Results of Blood Test (CBC)
  • Results of Chest X-ray
  • Results of Urinalysis
  • Results of Stool Examination
  • Consultation Fee

 

For Insurance Purposes

  • Medical Certificate Form from the Insurance Company
  • Certification Fee

 
HOW TO AVAIL OF THE SERVICE

Follow These Steps It Will Take You Please Approach
Register Client 

  
Interview client for the purpose of
securing medical Certificate.

     Advise client to comply first all the
requirements needed. Once
comlpeted,

Retrieve client’s old OPD record or make new record for new client.

Interview client for some personal
information needed.

Make note of  the date, time and the vital signs.
Refers client to the Physician on duty with the laboratory results attached to the OPD Record.

Nursing Attendant

 

Examination of Patient

Physician on duty assesses and examines the client before signing the certificate form.

 

5-10 minutes

Physician on Duty

  •  Payment of Fees

 

      Proceed to the cashier for payment
of required fees.

 

 

 

Cashier

 Issuance of Certificate

      Personnel accomplish certificate   
form and have it signed by the
physician.
